Use the form below to book your appointment with Oralei. If you are unsure as to which appointment type is best suited to your needs, please email hello@oralei.com.au and we will get back to you as soon as possible.

Please check your emails for an appointment confirmation. For initial visits please complete the form sent to you prior covering relevant health history.

Note deposits are required for all bookings. No show appointments or late cancellations will be charged a fee.
